Health Outreach Partners (HOP) is a national non-profit health organization providing training, consultation, and information services to community-based organizations striving to improve the quality of life of hard-to-reach populations.
Since 1970, HOP has been at the forefront of elevating the importance of outreach, recognizing the critical role it plays in facilitating case management, primary care, health prevention and social services to underserved communities. By professionalizing outreach services, HOP has established itself as the leader in enhancing health organizations' ability to successfully reach farmworkers and other vulnerable populations and offer critically-needed services.
