The Admissions Nurse plays a vital role in hospice care as the first point of contact for patients and families facing a terminal diagnosis. In this position, you will travel to patients wherever they reside, conduct comprehensive assessments, and gather essential clinical information to ensure timely and appropriate care. You will also provide compassionate education and guidance to families as they navigate the beginning of the hospice journey. Admissions Nurses develop individualized care plans, identify immediate and ongoing needs, and help support families with clarity, empathy, and professionalism. The trust and comfort you establish during these early moments lay the foundation for a meaningful care experience, positively shaping how families move through the process with dignity, understanding, and peace.

Qualifications

Admissions Nurse Requirements & Key Responsibilities:

  • Current Registered Nurse (RN) license in the state(s) of practice; multistate license accepted where applicable.
  • 1+ years of hospice/home care experience and 2+ years of overall nursing experience preferred.
  • Ability to pass background check, drug screening, and consumer report.
  • Valid driver’s license, insured reliable vehicle, and acceptable driving record.
  • Perform nursing care in accordance with Nurse Practice Act, professional standards, and hospice guidelines.
  • Complete initial and ongoing patient and family assessments.
  • Develop, implement, and update individualized Plans of Care in collaboration with the care team.
  • Provide direct patient care, including medication administration, symptom and pain management, and ADL support.
  • Educate and support patients and families while maintaining accurate documentation and HIPAA compliance.
