Full-Time Occupational Therapist Liaison
If you are an Occupational Therapist looking for a different type of Occupational Therapy position, this opportunity located at Samaritan Hospital and St. Peter's Hospital as needed is for you. This position is a member of the Complex Care interdisciplinary team and is responsible for ensuring patients transition to the next best site of care.
What You Will Do
- Directly co-treat patients in collaboration with the primary therapists, providing supplemental therapies.
- Utilize your clinical expertise in occupational therapy to provide education and resources to ensure patients remain in the least restrictive environment when transitioning from acute care.
- Serve acute care & complex care team, ensuring the no place like home approach is utilized in all appropriate scenarios.
- Ensure timely progression to the next site of care by implementing appropriate occupational therapy treatment to vulnerable in need.
- Responsible for educating case/care management teams, social work/utilization management services as well as the greater hospital system on functional status, DME options and alternatives to inpatient rehabilitation for appropriate patients in an effort to maximize community re-entry opportunities.
- Problem solve and collaborate on the most vulnerable and complex SIOH needs in the hospital
- Graduate of an approved Occupational Therapy program
- Current NYS Occupational Therapist (OTR) license and registration
- 1 year experience as Occupational Therapist required
- 5 years in healthcare environment, including health plans, hospitals, ambulatory or post-acute care preferred.
- Willingness to go to both St. Peter's Hospital if needs should arise
- Excellent verbal, written communication, and interpersonal skills.
- Strong project management skills.
- Ability to inspire and build confidence in others and to forge alliances and garner support.
- Strong analytical and critical thinking skills, including the ability to resolve complex administrative, technical or human situations.
- Anticipate and resolve a broad range of issues on multiple, sometimes unrelated, topics, to determine the need for process development and revision.
- Familiarity with standard desktop and windows-based computer system, including email, e-learning, intranet and computer navigation.
- Ability to use other software required to perform essential functions. Ability to utilize multiple electronic medical record platforms.
