We are seeking a Part-Time Physical Therapist to join our team in the Waterbury, CT area.
Position Summary
The Physical Therapist is responsible for evaluating, planning, and implementing individualized treatment programs for patients receiving home health services. Our patient population primarily consists of elderly Medicare and Medicaid patients recovering from hospitalization or managing chronic conditions.
This role offers flexibility, autonomy, and the opportunity to make a meaningful impact in patients’ lives.
Responsibilities
- Perform comprehensive PT evaluations in the home setting
- Develop and implement individualized treatment plans
- Establish functional goals focused on mobility, safety, and independence
- Perform reassessments and adjust care plans as needed
- Collaborate with RNs, PTAs, OTs, physicians, and office staff
- Complete timely and accurate documentation in the EMR
- Participate in interdisciplinary case conferences as needed
- Educate patients and caregivers on home safety and exercise programs
- Active Connecticut Physical Therapist license
- Minimum 1 year clinical experience (home health preferred)
- Strong documentation and communication skills
- Ability to work independently and manage schedule efficiently
- Reliable transportation and valid driver’s license
