Design Professional Services – Criteria Architect

The Fredericktown Local School District Board of Education (“Board”) is soliciting Statements
of Qualifications from qualified Design Professionals to provide criteria architect services
for proposed improvements to the District’s athletic stadium and fieldhouse, including
potential upgrades, renovations, and/or new construction (the “Project”).
The Board intends to utilize the design-build project delivery method as permitted under the
Ohio Revised Code. The selected design professional will assist the District in establishing
design criteria and related documentation necessary for the design-build process.
Scope of Services
The criteria architect will provide professional services consistent with the requirements of the
Ohio Revised Code and Ohio Administrative Code governing design-build projects, including but

[RFQ(s] for selection process of a Design-Build firm for the identified project
types and support the evaluation committee in the determination of a ‘short-list’
for the Request for Proposal phase
○ Assist in development, preparation and issuance of Request for Proposals
[RFP’s] for selection of a Design-Build firm for the identified project types and
support the evaluation committee in the determination of a recommendation.
○ Assist in completion of any required filings or submittals required for
Interested firms should include the following regarding their qualifications:
oProject owner, name of project and location;
oBrief description of the project;
oYear services were completed or anticipated completion date;
oOther relevant information about the project and the firm’s services;
oReference contact person and phone number;
Submission Requirements
Interested firms should submit three (3) copies and one (1) electronic copy (PDF) of their Statement of Qualifications, prepared in accordance with Ohio Revised Code Sections 153.65–153.71, to:

Interested firms should include the following regarding their qualifications:
- Experience with similar projects with the Board;
- Information about the firm’s history;
- Education, technical training, and experience of owners and key personnel;
- The firm’s experience in providing substantially similar services in Ohio (i.e., design services for public schools and specifically athletic facilities);
- Ability of the firm to provide services as needed by the Board based upon the firm’s current volume of work;
- List of consultants used to provide any services not performed by the design firm;
- Past performance as reflected in references from previous and current clients for which the firm has provided or is providing similar services; please include a list of 5 relevant projects involving similar services performed by the firm during the past 5 years.

oYear services were completed or anticipated completion date;
oOther relevant information about the project and the firm’s services;
oReference contact person and phone number;
- The firm’s location and proximity to the site for purposes of attending meetings at the Board’s facilities and observing work in progress;
- The ability of the individuals identified by the firm who will be responsbile for providing servcies to communicate with the Board and its administrators.
