About the role

As Pathway Coordinator, you will support our adult residents, enabling them to build independence, achieve planned outcomes, and progress along a clear pathway to successfully move on to independent living.


You will be responsible for coordinating and managing each individual’s support pathway, ensuring all interventions are outcome-focused and aligned to progression goals. The Pathway Coordinator is also responsible for ensuring all support activity is clearly evidenced, including accurate, timely and outcome-focused recording on systems such as In-Form (or equivalent), and for maintaining high standards of professional practice.


We are seeking someone who has experience working with vulnerable adults and delivering person-centred support. You will need strong communication and organisational skills, resilience, and a compassionate, person-centred approach to working with individuals who may face complex challenges.
