We’re now seeking a Dementia Care Partner to join our team, servicing clients across the Hills area and Northern region (including Northern Beaches, Lane Cove, Ryde, Hornsby, Pennant Hills, Carlingford and Castle Hill.
Your Opportunity
Servicing clients across Northern Beaches / Hills Areas
Part Time | Monday to Thursday | 30.4 hours per week | 18‑month contract
Hours: 8:30am – 4:30pm or 9:00am – 5:00pm
This is a community based role supporting people living with dementia and their families through education, guidance, advocacy and tailored support.
You’ll work closely with clients in their homes and in community settings, helping them navigate services, understand dementia and maintain quality of life.

In This Role, You Will
- Provide person-centred support, advice and advocacy to clients living with dementia
- Conduct assessments and develop care plans to support individual needs
- Confident in presenting Dementia Education within Anglicare and the Community
- Deliver education and psychosocial support to families and carers
- Support clients to access services and navigate the aged care system
- Monitor client progress and adapt support as needs change
- Maintain accurate documentation and meet program KPIs
- Build strong relationships with families, clinicians and community networks
You’re empathetic, confident and skilled at working with people experiencing complex challenges. You bring both compassion and practical problem-solving to your work.
You Will Also Bring
- Experience in aged care, community or dementia related roles
- Strong communication skills and the ability to build trust with clients and families
- Understanding of dementia and its impact on individuals and carers
- Experience with assessments, care planning or case management
- Relevant qualification in Community Services, Dementia Care or similar (essential)
- Current driver’s licence and access to a vehicle
