Summary Of Position
- The Community Health Outreach Supervisor oversees the design, coordination, and implementation of community-based programs that advance ACPNY’s population health, quality, and health equity goals.
- Supervises a team of Community Health Workers, providing day-to-day supervision, coaching, performance guidance, and professional development to ensure high-quality, culturally responsive patient outreach and engagement.
- Ensures Community Health Worker activities align with program goals, care coordination workflows, and regulatory or grant requirements, while monitoring workload, productivity, and outcomes.
- The Community Health Outreach Manager oversees community-based programs that support ACPNY’s population health and quality goals.
- Monitor performance and outcomes by using data to evaluate Community Health Worker productivity, outreach & screening effectiveness, and program impact, driving continuous improvement.
- Supervise, coach, and support Community Health Workers to ensure effective, culturally responsive screening, navigation and patient engagement
- Collaborate with internal stakeholders to integrate social needs and interventions into care delivery workflows.
- Conduct and manage Social Determents of Health (SDOH) screenings, ensuring all results are accurately completed and uploaded to the Unite Us (referral) platform.
- Build and maintain relationships with community organizations to support patient needs and expand resource connections.
- Partner with clinical and operational leaders as it relates to the community-based program goals.
- Ensure Program and Regulatory Compliance with Social Care Networks and other community partners.
- Monitor Community Health Worker’s activities and workflows to ensure alignment with program standards, regulatory requirements, and grant obligations.
- Support reporting and documentation to ensure accurate an level of achievement of outreach activities, outcomes, and program metrics for internal reporting and audits.
- Identify opportunities to enhance community health initiatives and support ACPNY’s value-based care and health equity strategies.
- Performs other job-related duties as required.
Education, Training, Licenses, Certifications
- Bachelor’s degree — Public Health, Health Sciences, Social Work, Human Services, Health Care Administration, or related field.
- Master’s degree - Public Health (MPH), Public Policy, Social Work (MSW), Health Administration (MHA), or related discipline.
- 3–5 years of experience in community health, population health, care management, public health, or related patient-facing roles.
- 1–2 years of supervisory or team-lead experience — particularly overseeing Community Health Workers, outreach staff, or similar frontline roles.
- Experience working with underserved or diverse populations.
- Experience with value-based care, quality improvement, or health equity initiatives.
- Prior work within healthcare, ambulatory care, or social services settings.
