
Schedule: Monday - Friday 8:30am-4:30pm
Duties & Responsibilities
- Provide assessment of newly referred patients and assess health needs in home care setting
- Work with patient to create a patient centered plan of care
- Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family
- Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services
- Provide supervision and training of PCA/HHA staff
- Assist with the on-call rotation (stipend provided)
- NYS Licensed Registered Professional Nurse
- Minimum of Associate's Degree required
- One year Nursing experience required
- Home care experience a plus

The pay range displayed on this job posting reflects the anticipated range for this position. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set, and education.
