TransForm SSO is more than just information technology, clinical solutions and project management.
We deliver exceptional customer service to our clients and help improve value to Ontario’s healthcare system. Our business model and strategic approach is rooted in exceeding our members’ needs and centres on service, performance, innovation and people.
We provide services to the five hospitals in the Erie-St. Clair LHIN and also manage exciting projects like the Health Links initiative and connecting South West Ontario (cSWO), both of which will help improve patient care in Ontario.
Founded in 2013, following the amalgamation of PROcure Healthcare and Consolidated Health Information Services, our commitment is to deliver exceptional service and create new opportunities to improve value to the health system.
