WHAT WE DO: I help families, social workers, case managers, and physicians achieve a smooth transition for clients who need in home care by providing a comprehensive assessment to develop a plan of care for those who need assistance at home, whether it’s with a home health aide, CNA, RN, PT or OT in Palm Beach and Broward County.

WHAT MAKES US DIFFERENT? Unified Pledge Home Health is a clinician-owned and operated company. Maria Huling (me!) is the director of nursing and owner. We are a husband and wife owned company providing 5 STAR care.
HOW IT WORKS: Whether the planning starts at the facility where the client will be discharged from, or in the client home, an RN will do an evaluation based on the client’s needs. Once the RN determines the plan of care, coordination of care will be implemented. With Family involved, and great communication, the transition of home care becomes a seamless experience.
