Breast & Cervical Cancer Program (BCCP) -IDOH ongoing grant coordinating the breast care coverage of 27 counties

Other Grants: Secret Sisters, HeartStrings, Pink Ribbon and others
Community Services with All Deaf
Contract sign language interpretation with certified interpreters across northwest Indiana and southwest Michigan
Community events for the deaf and awareness
ADA compliance assistance for the deaf and hard of hearing
Covering Kids and Families of United Health Services
Team of navigators, certified by the IN Dept of Insurance
All state insurances and the Marketplace St. Joseph, Elkhart, Marshall counties

Navigation on-site, at our office and over the phone
Diabetes Resource Center
Small program which can assist with immediate need of insulin for qualified people
250% or lower, lives in Indiana, exhausted other options
Assistance with patient assistance programs & discount cards / programs for meds
Referrals to appropriate assistance
Indiana WISEWOMAN
IDOH ongoing grant, must be a BCCP enrollee 40-64 uninsured
Cardiac and diabetes screenings at the providers’ offices
UHS supplies the Healthy Behavior Supports and physical activity options
Health First Grant
SJ County Health Dept limited grant, ending 12/31/2024
UHS supplies the Healthy Behavior Supports and physical activity options
Suicide Prevention Center
Brings awareness and suicide prevention along with survivor support to the community
Living Works, QPR and ASIST training available for a small fee
